Replacement Cards

Replacement Induction Cards.

In order to obtain a replacement General Safety Induction Card through Broadview Safety Pty Ltd you must have completed the National "WHITE CARD" course with us, OR have a Statement of Attainment from the Registered Training Organisation (RTO) you did the course with.

There are two ways you can obtain a replacement card:

1)  Come into our office during office hours with:

  • Photographic Identification

  • A Statutory Declaration stating your card has been lost, stolen, mislaid, destroyed or another reason and the circumstances involved.
  • The applicable fee.
and you will be issued with a replacement card on the spot.

2)  Contact us.  We can post/fax/e-mail you an:

  • Application Form
     
  • A Statutory Declaration
both of which need to be completed in full and sent back to us with the applicable fee. 

Cost: $30.00 per Replacement Card.
