Enrolment Form
This Enrolment Form is to be completed by those wishing to obtain a Machinery Licence.
It is a necessity that this form be completed by each person.
It must also be accompanied by a Training Plan before any Training can commence and Training Material (including Log Books) can be released.
Please return both forms to our office along with any applicable deposit payment.
For your convenience, this can be returned by post, fax or e-mail.
For other courses we offer, this form is issued and completed on the day of the course.
